Elevance Health supports a hybrid workplace model with pulse point sites (major offices) used for collaboration, community, and connection. This position can be primarily virtual, however, will be based within 50 miles of a pulse point site listed above.

The territory for this position is in and around Augusta, Georgia. Therefore, the selected candidate must reside in Augusta, GA.

Hours: Monday - Friday 9 am - 5:30 pm EST.

The Community Engagement Navigator is responsible for engaging with members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between the company and referral sources.

How you will make an Impact:

  • Engage with members who have been identified as possessing significant SDOH barriers. Work with the member to establish goals and next steps for addressing SDOH.

  • Support members in selecting providers or programs most appropriate for supporting their needs.

  • Interface with HCM to ensure that the individualized plan is aligned with existing or needed case management efforts.

  • Act as a liaison between referral sources and the health plan.  

Minimum Requirements:

  • BA/BS degree in Public Health, Social Work, Rehabilitation Counseling, or related field and a minimum of 2 years experience with marketing, job development, job placement, or social work; or any combination of education and experience that would provide an equivalent background.

Preferred Skills, Capabilities, and Experiences:

  • Knowledge of Voc Rehab preferred.

  • Travels to worksite and other locations as necessary.

  • This role necessitates local driving in and around Augusta, GA.

  • Bilingual skills are a plus.

  • OB experience is a plus.

  • Community Health Worker Certification is a plus.

  • Excellent verbal and written communication are strongly preferred

  • Ability to meet necessary internet speed requirements and possess PC Skills.
