Clinical Program Consultant
Remote Monday- Friday with some travel to provider offices, must reside in Louisiana
This is an exciting opportunity to join a multidisciplinary team and work with primary care and behavioral health providers to ensure that member's psychosocial and whole health care needs are met. This positions will be ensuring that evidenced based clinical practices are delivered by Healthy Blue's provider network and that HEDIS gaps are closed. Further will help to develop community based linkages between providers to make transitions of care easier for our members. This is a work from home position with travel requirements. Responsible for providing clinical program and project support in a Medical Management department supporting Healthy Blue network Providers engaged in Value Based Programs.
How you will make an impact:
- Develops and implements clinical programs for the management of members with specific medical conditions. These programs emphasize whole Person Care and the integration of Physical Health and Behavioral Health
- Serves as a project manager for such medical management initiatives, such as medical review redesign, development of policies and procedures, development and implementation of training procedures and manuals, and the development and implementation of metrics and other reporting capabilities. Providing educational detailing to health plan providers
- Represents the department on enterprise-wide and regional committees and program initiatives. Requires a BS in nursing or related medical management area and minimum of 5 years project/program management experience; or any combination of education and experience, which would provide an equivalent background.
- Requires a BS in nursing or related medical management area and minimum of 5 years project/program management experience; or any combination of education and experience, which would provide an equivalent background such as licensed clinical psychologist, senior licensed clinical social worker.
