LTSS Service Coordinator - Service Initiation

This position will serve TN Members virtually, working from your home-based office. TN residency required.

This LTSS Svc Coordinator role will function primarily as a Service Initiation Specialist (SIS) responsible for providing collaborative assistance to Coordination teams and related stakeholders in reducing Service Initiation Delays by engaging in meaningful communication with providers to partner on innovative solutions that result in timely initiation of services.  The SIS will also collaborate with stakeholders to support initiatives around Workforce Development and Provider Network optimization.  This position supports the current ECF SIS as back up when needed, and will also complete all tracking, trending, and reporting of all CHOICES service initiation data for internal and external deliverables.

Primary duties may include, but are not limited to:

  • Communicates progress, issues, and barriers to internal stakeholders.

  • Facilitates monthly meetings with internal stakeholders

  • Researches, compiles, extrapolates and reports data related to service initiation operational needs.

  • Collaborates with provider through various outreach avenues for service initiation needs.

  • Monitors internal reporting databases, ensuring adequate and updated service initiation member status.

  • Collaborates with Referral Coordination and Care Coordination teams for service imitation needs.

  • Participates in project initiatives supporting service initiation.

Qualifications and Skills

  • Requires a BA/BS in a related field or 3+ years  managed care experience; or any combination of education and experience which would provide an equivalent background.  

  • Specific experience working with ECF CHOICES and/or CHOICES; Long Term Supports and Services (LTSS) in Tennessee, highly preferred.
