Location - Remote (Candidates must reside in CT)

Responsible for care coordination and/or care management activities focused on the Wellness and Recovery of Beacon members. The Peer Wellness and Recovery Specialist works as a member of an inter-disciplinary team to implement and monitor treatment for individuals being served within behavioral health settings, those in the community or residential treatment or outpatient facility.   This position involves participation and implementation of program goals and objectives to promote a strengths-based, wellness-informed client centered approach to assisting individuals being served in achieving person-driven wellness and recovery goals.  

Primary duties may include, but are not limited to:

  • Identifies opportunities for engagement of members and their families in forming a supportive, recovery network.

  • Develops and implements provision of onsite psychiatric discharge planning education at Recovery and Resiliency sites.

  • Collaborates with Stabilization Teams as a member advocate in discharge planning education, resolution of barriers, and service transitions.

  • Acts as a resource for staff on decision making and problem solving.

  • Initiates and maintains contact with assigned individuals and providers to determine member's response to services. Provides therapeutic interventions with individuals being served as required, and directed by the behavioral health treatment plan and peer intervention protocol.                  

  • Provides individual training in living skills, creation of healthful lifestyles, and support in areas such as self-care, holistic wellness, hygiene, community resources, and other life skills as directed by the plan of care.                  

  • Participates in regular clinical supervision and attends clinical case conferences and staff meetings to gain knowledge and communicate program and individual needs served                 

  • Complies with and implements the regulations, policies, and procedures, which pertain to peer delivered service goals and rules.     

  • Respects and honors individuals’ rights and responsibilities, and demonstrates professional ethics and boundaries.                  

  • Reflects positive peer values that include non-hierarchy and mutuality when appropriate.                 

  • Documents and maintains clinical records in a manner that assures compliance with all agency policies, procedures along with all local, state, and federal regulations.  Maintains accurate and up-to-date documentation as required associated with assessment, treatment, and activities of individuals served.                  

  • Adheres to mandatory abuse reporting laws and HIPAA requirements.                 

  • Acts as a positive role model for individuals served, showing a recovery and healthy lifestyle orientation.                                 

  • Actively attends and participates in regular in-service trainings.

  • Acts as an advocate for individuals served when appropriate, both within the organization and with other entities.                 

  • The incumbent may represent the Beacon Peer Wellness Program at community, state, and /or national functions related to the work of the Peer Wellness Program.                    

Requirements:

  • Requires AA/AS and minimum of 2 years of experience in health services or behavioral health field or in a peer support services role; or any combination of education and experience, which would provide an equivalent background. Peer Specialist Certification required or obtained within 1 year of hire.

  • Knowledge of care-coordination and case management concepts strongly preferred.
