Mobile Based in Southern CA: Visiting members/patients in the City of Los Angeles, CA.

The Nurse Practitioner (Skilled Nursing Facility) will be responsible for providing healthcare services to patients under skilled care in nursing facilities, in collaboration with the attending physician, through advanced assessment and identification of health states including abnormal conditions, diagnosis, treatment and evaluation. Responsible for collaborating with rounding physician(s), the patient’s other physicians and providers, and their family members to develop complex plans of care in accordance with the patient’s health status and overall goals and values. 

How will you make an impact: 

  • Utilize evidence-based clinical practice standards to provide direct clinical services for patients within prescribed guidelines, within the scope of practice, and under supervision of a collaborating physician

  • Acquire patient health histories, performing physical examinations, ordering and interpreting complex diagnostics, and making critical decisions regarding a wide variety of health situations, including high risk and crisis situations

  • Perform health assessments, specialized risk assessments, and developing plans of care for patients facing acute and chronic and health conditions.

  • Make routine in-person (and virtual) visits to nursing homes as clinically necessary and in accordance with Medicare guidelines 

  • Document encounters precisely and timely on the appropriate EHR system, and clearly communicating plans of care and/or any changes of condition to patients and team members.  Formulating safe discharge plan with patients, families, and other interdisciplinary team members, completing discharge summaries and providing sign-out PCPs and other team members upon discharge

  • Will drive and conduct in-person visits to multiple skilled nursing facilities. 


Minimum Requirements:

  • Requires an MS in Nursing and minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background. 

  • Current unrestricted RN license and NP license in the state of California is required

  • Prescribing or furnishing authority/DEA licensure in good standing

  • For CareMore business units, satisfactory completion of a Tuberculosis test is a requirement for this position.

Preferred Skills, Capabilities and Experiences:

  • Experience working in skilled nursing facility.

  • Experience working with geriatric patient population.
