Location: Field Based - This position requires driving and in-person visits to patient homes in the Los Angeles County, CA areas (Pico Rivera, Montebello, and other surrounding cities).
Nurse Practitioner: Home-based Care
The Field Nurse Practitioner is responsible for providing in-person and virtual healthcare services to patients at home, in collaboration with the attending physician, through advanced assessment and identification of health states including abnormal conditions, diagnosis, treatment and evaluation. This team member will deliver a mix of home-based palliative care, chronic disease management for homebound patients, and higher acuity “Hospital at Home” care to complex patients. Responsible for collaborating with the patient’s other physicians and providers, and their family members to develop complex plans of care in accordance with the patient’s health status and overall goals and values.
How will you make an impact:
Utilize evidence-based clinical practice standards to provide direct clinical services for patients within prescribed guidelines, within the scope of practice, and under supervision of a collaborating physician
Acquire patient health histories, performing physical examinations, ordering and interpreting complex diagnostics, and making critical decisions regarding a wide variety of health situations, including high risk and crisis situations
Perform health assessments, specialized risk assessments, and developing plans of care for patients facing acute and chronic and health conditions.
Make routine in-person (and virtual) visits to patient homes as clinically necessary and in accordance with Medicare guidelines
Document encounters precisely and timely on the appropriate EHR system, and clearly communicating plans of care and/or any changes of condition to patients and team members.
Participate in Rotating On-Call program which may include after-hours, weekend and holidays
Requires an MS in Nursing and minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
Current unrestricted RN license and NP license in the state of California is required .
Prescribing or furnishing authority/DEA licensure in good standing
Must be flexible with local traveling within the assigned region for daily home visits and covering assignments for teammates in case of emergency.
For CareMore business units, satisfactory completion of a Tuberculosis test is a requirement for this position.
Preferred Skills, Capabilities and Experiences:
Minimum of 2-3 years of practicing Nurse Practitioner experience; experience with complex care.
2-5 years of experience working in an acute care settings such as ICU, CCU, telemetry, emergency medicine is a plus.
Proficient in use of EMR and other technology use.
