This position will serve members in New York virtually, working from your home-based office. Must live within commutable distance to our Lake Success NY Office.

The Care Plan Nurse has expertise in developing person-centered service plans (PCSP). This plan will reflect the individual’s needs, preferences, and goals in receiving services to maximize independence and community integration.  The plan will also incorporate social and cultural considerations. This role will also be responsible for participating in enrollment of potential members and the completion of other assessments for both potential and existing members. The Care Plan Nurse is a remote role working from home.

Primary duties may include but not limited to:

  • Participate in enrollment activities per organization and regulatory MLTC policies which includes completing enrollment documents

  • Review and utilize the Community Health Assessment (CHA) in HCS/UAS-NY on potential enrollees/existing members to develop the PCSP 

  • Assess potential enrollees/existing members physical, social, and mental health status, needs, preferences and abilities to develop the PCSP

  • Collaborate with the Care Management team as needed to assist in the development of the Person-Centered Service Plan

  • Review medications and diagnosis, update accordingly  

  • Review & utilize the CHA for members post Transitions of Care to develop a PCSP  

  • Complete other assessments and documentation, such as falls and home assessments, for initials, ad hoc and routine assessments 

  • Document clinical information in members EHR adhering to policies and procedures 

  • Scheduling member visits   

  • Ability to perform other duties as assigned 

Requirements:

  • Requires an RN and minimum of 3 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role; or any combination of education and experience, which would provide an equivalent background.  

  • Current, unrestricted RN license in applicable state(s) required.  

Preferred Qualifications

  • Bilingual - Spanish, Korean, Chinese (Mandarin, Cantonese), Russian, Bengali preferred

  • MA/MS in Health/Nursing preferred.  

  • May require state-specified certification based on state law and/or contract.  

  • Occasional travel or hybrid work option to Lake Success office may be required
