The Outreach Care Specialist (OCS) is responsible for serving as a liaison/connector between Target, Inc. employees and the community.  The OCS provides onsite vendor support within the Target, Inc. workplace and assists employees to navigate internal benefits and connect to community-based resources.  The OCS conducts in person, telephonic and/or video appointments to assess and provide support around social determinant of health needs such as health care, education, financial health, housing support and more.


Primary duties may include, but are not limited to:


•    Meets employees in assigned Target, Inc. locations to help identify social determinants of health needs impacting employee health and general wellbeing
•    Collaborates with the employee and care team to develop action plan
•    Develops trusting relationship with Target, Inc. employees by providing support and advocacy to help achieve health goals
•    Builds strong relationships with Target, Inc. employees and site leadership team 
•    Assists Target, Inc. employees in accessing health-related services and community resources, including assisting with applications, program outreach/coordination and following up to ensure needs are met
•    Documents interactions in electronic medical records and completes all other required documentation in a timely manner
•    Attends meetings, completes trainings and other deliverables within required timeframes 

Minimum Requirements:
•    Requires a high school diploma and minimum of 1-year of related experience; or any combination of education and experience which would provide an equivalent background

Bilingual Spanish Required


Preferred Qualifications:
•    Certified nurse assistance or certified medical assistant and/or BS/BA degree in a related field preferred
•     Experience in a community-based or social work environment or experience in Case Management Care Coordination and/or any combination of education and experience which would provide an equivalent background

  • For candidates working in person or remotely in the below locations, the salary* range for this specific position is $18.46 to $33.23
  • Locations:  Colorado; Nevada, Jersey City, NJ; New York City, NY; Ithaca, NY and Westchester County, NY
