Outreach Care Specialist (Waterbury/Torrington/Danbury)

The Outreach Care Specialist is responsible for creating and ensuring that appropriate member treatment plans are followed on complex cases and for proactively identifying ways to improve the overall health of our members and meet quality outcomes.

Primary duties may include, but are not limited to:

  • Creates Crisis and Plan of Care for families with children in crisis.

  • Coordinates follow-up care plan needs for members by scheduling appointments or connecting families to support and services.

  • Assesses member progress with mental health treatments and education support.

  • Identifies barriers to plan compliance and coordinates resolutions through a collaborative process.

  • Recommends treatment plan modifications and determines the need for additional services, in collaboration with the treatment and support team.

  • Coordinates identification of and referral to local, state, or federally funded programs.

  • Coaches youth and families on ways to reduce health risks, improve overall mental health functioning, and build self-advocacy skills.

  • Prepares reports to document files and compliance updates.

  • Facilitates intake and discharge assessment processes.

  • Document weekly family and collateral contacts.

  • Facilitate in-home youth and family planning meetings.

  • Learn and utilize the Wraparound Practice Model

Minimum Requirements:

  • Requires a high school diploma and a minimum of 1- year of related experience; or any combination of education and experience which would provide an equivalent background.

Preferred Skills, Capabilities, and Experiences:

  • BS/BA degree in a related field strongly preferred.

  • Bilingual Spanish is strongly preferred.

  • 2 years experience of working within the community strongly preferred.
