Transition Specialist

The Transition Specialist is responsible for preparing members with Serious Mental Illness (SMI) for their transition into independent community living pre and post-relocation. Assists members with facilitating independence in community living by providing education; identifying tools and resources to perform self-care using environment modifications; and engaging with community partners to build strategy with local stakeholders. Advocates on behalf of the member, helping resolve case or broader systems issues. Supports service coordination teams and connects members with those who can provide insight and expertise to assist in overcoming challenges.  

Primary duties may include but are not limited to:

  • Coordinates with the Service Coordinator and nursing facility to develop a transition plan.  

  • Provides intensive psychosocial rehabilitative support, initiating therapeutic interventions as needed.  

  • Conducts cognitive adaptation training and other evidence-based skills training for members.  

  • Conducts evaluation assessment interviews with members regularly. Performs periodic on-site visits with members as needed.  

Minimum Requirements:

  • Requires a BS/BA in Health, Social Services or related field.  

  • A valid state driver's license may be required.  

  • Must reside in Austin or San Antonio, TX.

  • Local travel may be required.

Preferred Qualifications:

  • Previous experience in behavioral health, social work, or the transition of individuals from institutional settings into the community strongly preferred.
