Outreach Care Specialist Sr
Location : Must reside within 50 miles of Louisville, KY ; Shelbyville, KY ; Baton Rouge, LA ; or Metairie, LA.
Hours : Monday - Friday, 8 am - 5 pm EST.

As an Outreach Care Specialist Sr , you will be responsible for ensuring that appropriate member treatment plans are followed on complex cases and proactively identifying ways to improve the health of our members and meet quality goals. The Outreach Care Specialist Sr will be in the community, working collaboratively with formal supports, service providers, and informal networks in pursuit of a person-centric holistic approach to care. You will support the Whole Person Serious Mental Illness program through a longitudinal case management approach which includes interventions tailored to the specific needs of the member delivered over time, while creating long-term relationships with members.
How you will make an impact:
- Work with the members and their supports to meet the member’s goals.
- Participate in targeted interventions for their physical health, behavioral health, and health-related social needs.
- Focus on long-term care planning, member education, coordination of services, and a schedule of focused follow-up.
- Coordinate follow-up care plan needs for members by scheduling appointments or enrolling members in programs.
- Assess member compliance with medical treatment plans via telephone or through on-site visits.
- Identify barriers to plan compliance and coordinate resolutions.
- Identify opportunities that impact quality goals and recommend process improvements. Recommend treatment plan modifications and determine the need for additional services in conjunction with case management and provider.
- Coordinate identification and referral to local, state, or federally funded programs.
- Coach members on ways to reduce health risks.
- Prepare reports to document case and compliance updates.
- Establish and maintain relationships with agencies identified in appropriate contracts.
- Provide education to members regarding all services available.
- Participate in cross-functional teams on projects, initiatives, and process improvement activities.
Minimum Requirements:
- Requires a high school diploma and a minimum of 4 years related experience; or any combination of education and experience which would provide an equivalent background.
Preferred Skills, Capabilities, and Experiences:
- CNA or CMA preferred
- BS/BA degree in a related field preferred
- Bilingual candidates preferred
- Experience working with Medicaid preferred
- Experience working in Behavioral Health field
- For URAC accredited areas, the following professional competencies apply: Associates in this role are expected to have strong oral, written and interpersonal communication skills, problem-solving skills, facilitation skills, and analytical skills.
