Nurse Practitioner Specialty, Nephrology - Medical Office, CA 


Work Site Locations: 2444 W. Beverly Blvd., Montebello, CA And 10000 Lakewood Blvd., Downey CA 


*Will see patients at the two listed Carelon Health Medical Office locations and other offices as needed.


Work Shift: 5 Days a Week, Monday – Friday: 8:00 am - 5:00 pm


The Nurse Practitioner Specialty, Nephrology is responsible for providing for the health care needs of patients in collaboration with a physician, through advanced assessment and identification of health states including abnormal conditions, diagnosing, treatment therapies and/or evaluation. Performs care in the specialty area of nephrology and patients on dialysis. 


How will you make an impact: 

  • Utilizes evidence-based clinical practice standards to provide direct clinical services for patients within prescribed guidelines and physician instructions. 

  • Acts independently to perform physical assessments, diagnostic functions, specialized risk assessments, and make critical decisions regarding a wide variety of health situations, including high risk and crisis situations. 

  • Acquires patient health histories and evaluates patient, performing physical examinations. 

  • Orders, interprets and evaluates diagnostic tests to identify and assess patients clinical problems and health care needs. 

  • Discusses case with physician. 

  • Formulates and documents care plan. 

  • Prescribes medication, therapies and/or other forms of treatment. 

  • Evaluates the patient’s response to therapy. 

Minimum Requirements:

  • Requires an MS in Nursing and minimum of 2 years of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background. 

  • Current, unrestricted RN license and NP license in the state of California is required. 

  • Certification in a specialty or minimum of 1 year experience in the specialty area of Nephrology. 

  • For Paragon and Carelon Health business unit, satisfactory completion of a Tuberculosis test is a requirement for this position and Hepatitis B vaccine or signed waiver.

Preferred Skills, Capabilities and Experiences:

  • ANCC and/or AANP Board Certifications highly preferred. 

  • Bilingual communication skills in English and Spanish.

  • Experience in either Primary Care or an Internal Medicine setting with adults and geriatric patient population.


For candidates working in person or remotely in the below locations, the salary* range for this specific position is $132,756 to $199,134.
