The Outreach Care Specialist shall provide individual and family support to ensure members are connected to community services and resources and have the necessary supports.  The Outreach Care Specialist is responsible for promoting clear communication amongst a care team and treating clinicians in supporting the individuals and families.  Additionally, the Outreach Care Specialist is to coordinate member specific care plans along the continuum.

Essential Duties and Responsibilities:

 1.       Telephone and/or face to face support to members and families to offer care coordination, support and information. Members may need help navigating the system, completing applications (i.e. Voluntary Services, SSI, System of Care, DDS), locating non –traditional supports, and learning about self-advocacy skills.

2.      Participate in a Local Area Team approach to care management to enhance member treatment by identifying resources, access to care, identifying barriers to care and gaps in service.

3.      Work closely with the Care Management and Peer staff to coordinate services as well as community-based systems of care by leading the facilitation of the Wrap Around Care Coordination model.

4.      Outreach Care Specialists will make home visits; meet with parents; enter documentation in computer system; search databases for appropriate resources.

5.      Collaborate with the Department of Developmental Services, Department of Children and Families, Department of Mental Health and Addiction Services, and the Department of Social Services as necessary to support members.

6.      Build, develop and maintain a positive and collaborative relationship with providers and support organizations by establishing a local presence and by attending meetings.

7.       Outreach to, and work with family and individual advocacy groups across the state.

8.      Work with diverse teams that require independent work initiatives.

 Minimum Qualifications:

 

·         Education: Bachelor Degree in Education, Psychology, Social Work, Nursing, Business, Public Health, Health Care Administration or related field.  Proficiency in the Spanish language desirable.

·         Licensures:  N/A

·         Relevant Work Experience: Experience working in the educational or community system with ASD populations or lived experience.  Some combination of the following experiencies required. Experience with Outlook applications and Teams/ZOOM platforms. Good communicational and organizational skills. Minimum computer skills a plus.
