Wellness & Recovery Specialist

Location: This is a virtual/remote position working from your home. Ideal candidates will live within 50 miles of one of our Elevance Health PulsePoint locations.

The Wellness & Recovery Specialist is responsible for care coordination and/or care management activities focused on the Wellness and Recovery of  members.

How you will make an impact:

  • Identifies opportunities for engagement of members and their families in forming a supportive, recovery network.

  • Develops and implements provision of onsite psychiatric discharge planning education at Recovery and Resiliency sites.

  • Collaborates with Stabilization Teams as a member advocate in discharge planning education, resolution of barriers, and service transitions.

  • Acts as a resource for staff on decision making and problem solving.

  • Initiates and maintains contact with assigned individuals and providers to determine member's response to services.

Minimum Requirements:

  • Requires AA/AS and minimum of 2 years of experience in health services or behavioral health field or in a peer support services role; or any combination of education and experience, which would provide an equivalent background.

  • Peer Specialist Certification required.

Preferred Skills, Capabilities and Experiences:

  • BA/BS preferred.

  • Knowledge of care-coordination and case management concepts, strongly preferred.

  • Experience in health coaching and motivational interviewing techniques, preferred.

  • One year completed as a certified Peer Support Specialist or have equivalent related experience, preferred.

  • Telephonic customer service skills, strongly preferred.

  • Experienced in the recovery process and in facilitating recovery through established mental health processes, preferred.

  • Experienced in teaching and engaging clients in basic problem-solving strategies for self-directed recovery, preferred.

  • Ability to recognize signs and symptoms of mental illness and substance use disorders, preferred.

  • Experienced in utilizing community resources for independent living and teaching these skills to individuals with severe mental illness, preferred.

  • Experienced in establishing and sustaining self-help and educational groups by incorporating input from mental health consumers, preferred.

  • Proficiency using multiple computer platforms simultaneously with excellent typing skills and attention to detail, preferred.
